Journey To Wellness Staff
For patients with chronic health conditions, this proven program pairs participants with a wellness coach and a Summit membership for 3 months.  Individualized and group exercise classes, Registered Dietitian involvement, cooking classes and support groups are included.  Participant outcomes show increased physical activity, pain reduction, weight loss, improved quality of life and success with personal goals.

How the Program Works:

Once a physician's referral is acquired, the member is paired up with one of our wellness coaches. Together, they establish a personalized wellness vision and goals, develop an exercise program, and identify potential barriers to success and brainstorm strategies to overcome them. Participants may choose to exercise independently, or attend supervised exercise classes. Follow-up coaching sessions are scheduled according to each person's individual needs.

Program Length: 3 to 6 month program including Summit membership

Staff: Our multidisciplinary staff includes a physician, clinical exercise physiologists, dietitian, nurses, certified wellness coaches, fitness instructors, personal trainers and a respiratory therapist.
