Clinical dietitians provide nutrition services on all patient care units. All patients admitted to the hospital are screened for nutritional risk. Once a patient is identified, the dietitian conducts a nutrition assessment and determines an appropriate nutrition care plan.
Nutrition status is monitored over the course of hospitalization. All patients receiving tube or intravenous feedings are monitored to ensure adequacy and to make recommendations for advancement or modification of the feeding. The dietitians attend unit rounds with the hospital staff and are involved in discharge planning.
